I have successfully implemented this form although I have it in a separate database, not DOMCFG.nsf. The only thing that springs to mind is the question of security. In the ACL of your DOMCFG database anonymous (or default if you haven't put anonymous into the ACL) must have Reader access with "Write public documents" selected. The second thing to check is that you've selected "Available to Public Access users" in the form's properties (security tab).
